Membership Term: The membership becomes effective immediately upon enrollment and remains active for a period of 12 months from the date of sign-up.
No Refunds: No refunds will be given in the event a Member terminates the Plan prior to the end of the plan year.
Full Payment: The annual membership fee must be paid in full prior to any exam or treatment.
Non-Transferable: Membership benefits are not transferable, have no cash value, and may not be redeemed for cash.
Not Insurance: This is not an insurance plan and is not subject to regulation by the state Department of insurance.
No Combining Benefits: Plan membership cannot be combined with current dental insurance plans or other promotions and discounts.
No Insurance Claims: No insurance claim will be filed for Members under this Plan.
Individual Use: The Plan is for individual use only. It is not a group benefits plan.
Additional Family Memberships: Each additional family membership must be paid at the time of the initial membership or at renewal time. $50 Discount for each additional family member added.
Annual Fee Adjustment: Membership fee may be adjusted annually.
Member Responsibility: Members are responsible for notifying the Dental Practice of any address or contact changes.
Missed Appointment Fees: Missed appointment fees/penalties are not eligible for the membership discount.
Payment at Time of Service: The fee discount will be void if full payment is not received at the time of service.
Exclusions: Ortho Services, CT scan and Panoramic x-rays are excluded from the Plan.
Payment Methods: CareCredit or Cherry cannot be used to make payments towards the Plan.
Missed / Canceled appointments without at least 48 hours’ notice will result in the forfeiture of the scheduled services.
